Basic Plan

Premium Plan

  • Complimentary contact lens training

  • Includes additional routine vision for prescriptions updates
  • Complimentary 6-month prescription check and change
  • Complimentary expedited shipping on contact lenses
  • Complimentary trial contact lenses to supplement supply as necessary
  • 1 Complimentary Low Level Light therapy session per year
  • Discounts on all materials and products

Restrictions and Exclusions (Important!)

  • Excludes medically oriented office visits, testing and exams
  • Excludes Medicare or Medicare Advantage
  • May not be combined with other vision insurances or discount plans
  • Must have a doctor’s clearance for light therapy
  • Family plan upgrades are for immediate family members at current pricing
  • Discounts only apply to services and products directly provided by Southwest Vision
  • Outside prescriptions or third-party lab orders are not permitted

Renewal & Cancellation Policy

  • Auto-renews annually (can apply unused banked money to re-enrollment fee)
  • Autopay monthly to add to your bank
  • Written 30-day cancellation notice to info@southwestvision-austin.com
  • No refunds for unused benefits or member bank dollars
  • Membership benefits are non-transferable and only available to member enrolled

Member Responsibilities

By enrolling, you agree to:

  • Keep your contact and billing information current.
  • Schedule appointments in advance when possible.
  • Present valid identification for membership verification when redeeming benefits.

Practice Rights

Southwest Vision reserves the right to:

  • Adjust membership pricing or benefits upon renewal.
  • Terminate membership for misuse, non-payment, or violation of terms.
  • Modify the Program with 30 days’ notice.
